A Coordinated Care Organization
A Coordinated Care Organization (CCO) is a community based organization made up of all types of care providers who come together to support the health of the community. This approach will make receiving healthcare in Oregon far more straightforward. Patients will have one point of contact for most of their healthcare needs, and providers will be able to better coordinate care. CCO's are created to achieve the triple aim of improving health, quality of care, and lowering the costs
Prior to the formation of Coordinated Care Organizations, services such as behavioral and physical healthcare were offered separately, often in fragmented and uncoordinated ways, so that members may have had gaps in their care. Providers were paid for treating illness, not for preventing it. Individuals with chronic conditions may not have always received services to avoid unnecessary hospitalizations or emergency care.
By utilizing community health workers to work with patients with chronic illnesses, CCO's will improve health outcomes and save money by ensuring these patients are taking the appropriate medications and managing their conditions.
The cost savings we anticipate for Oregon are substantial -- more than $3 billion over the next five years -- and will help our most vulnerable members maintain coverage, while freeing limited resources for other public priorities.

National Committee for Quality Assurance
The National Committee for Quality Assurance has awarded an accreditation status of Accredited for service and clinical quality that meet the basic requirements of NCQA's rigorous standards for consumer protection and quality improvement.
